****Due to the Covid-19; we have taken extra pre-cautions to ensure the safety of our staff and patients. In order to cover the cost of necessary PPE there may be an additional charge. We are requiring the additional steps:

• patient to come by his/herself unless an additional person is needed/required (i.e: transportation, minor)
• a verbal screening will be done over the phone. There will also be an in-person screening and temperature taken upon entry 
• we recommend our patients to arrive in a mask, if possible as recommended by CDC
• we are temporarily closing our waiting room, so we ask when patients arrive to remain in their vehicle and give us a call/text
• Patients should fill out their paperwork by printing them out from our website and either emailing or bringing it to their appointment to reduce contact to common surfaces
